He was a very pleasant person. He did ask a lot of questions. We discussed my diving history. And in the end, he did a great job getting my paperwork to me. I will use their services again next year.
Everything went great. Doctor thoroughly went through my recent medical history and asked relevant questions regarding my dive history and any problems that could arise. Booking the appointment was very easy and worked well with my schedule. Would use again
I had a great experience getting my medical done. I was pursuing a professional level certification in scuba, and with complications in local facilities being willing to do the physical getting this done was a huge weight off my shoulders. Now officially a dive instructor.

Dr Adam Kawalek signed my dive medical form recently and he was absolutely amazing. I had some sensitive information to discuss regarding my prescription and he answered all of my questions and was very knowledgeable. It was super easy to set up the next day appointment. I highly recommend dr Kawalek and Dive Slip!
Dr Kawalek was personable, professional and answered all my questions before I went on our dive trip. I would highly recommend him and it was great to be able to schedule an appointment so quickly.

The doctors was easy to talk to and explained as he went along, and when I had any questions. I filled out the required forms, and had my clearance to dive completed that very same day, in fact within the hour. I feel more confident knowing I am ready to dive on a medically safe level. Be honest with him, he is honest with you. I would definitely refer others.
Having an online service to get a quick Doctor visit to see if you are clear to dive is amazing. Especially since one usually has to wait a few weeks to get a Doctor appt.
Addressed my concerns and got me my dive clearance form in about 24 hours. Would use again!
